2025-26 Athletic Physical Forms Update:
We are excited to announce that we are now a full partner with BOUND, a full-scale athletic, activities, and facilities management system. Most of you already look to BOUND to see high school schedules, scores, and standings, however now we are using BOUND as our main site for all our athletic schedules (JH to Varsity), school calendar, student physical forms, and reserving our facilities.
Moving forward, the only paper forms that you will need to fill out by hand and upload online are the Preparticipation Physical Exam ( 2 pages - every 3 years from a licensed doctor) and the Interim Health History Form (1 page - in the off years when physical is not needed). The rest of the 5 forms are agreements that you will e-sign every year on the BOUND website.

**If you are not sure if your child is due for a Preparticipation Physical Exam this year, please contact Sue in the office or Alexa.

As with anything new, there will likely be some kinks that we will need to iron out, however, we are excited to have a one-place shop for you all to look to when needed.
